Coordinated care and support
PACE
The Program of All-Inclusive Care for the Elderly coordinates medical care and supportive services for eligible adults who want to remain safely in the community.
What PACE may include
- Primary and specialty medical care.
- Prescription medications.
- Transportation to approved services.
- Therapy, adult day services, meals, and social support.
- Home care and caregiver coordination when authorized.
General eligibility framework
PACE programs generally serve people age 55 or older who live in the program’s service area, meet a nursing-home level of care determination, and can live safely in the community with PACE support. Final eligibility is determined by the PACE organization and applicable state requirements.
PACE is not the same as a standard Medicare Advantage plan. Enrollment typically changes how covered services are coordinated, so a complete explanation is important before enrolling.
